Does anyone have any of the following data by state annually over the
1985-2005 period:

1 - number of physcians per state;
2 - number of hospitals per state;
3 - number of nurses per state;
4 - number of hospital personnel per state;
5 - average cost of a day of hospitalization per state

Any of the above would be greatly appreciated.
